All services: $125.00/hr
I am able to accept the following forms of insurance:
* Anthem BCBS
* Cigna
* Harvard Pilgrim/UBH
* NH Medicaid
Any required co-pay is due at time of service.
All clients are expected to give 24 hour notice by phone if intending to cancel. If there is less than 24 hour notice of a cancellation, a $60.00 fee will be charged to the client. This is due at the next session.
